Plan differences:  

•  12‑Month Membership Best for complex or long‑standing conditions and those wanting long‑term support. Up to 6 visits (1 hour each) are included with this plan. 
• 6‑Month Membership Ideal for a focused phase of care around a specific goal.  Up to 3 visits (1 hour each) are included with this plan

For patients continuing care after initial 6 or 12 month plan. 

• Concierge service includes unlimited messaging with your provider
• Up to 4 visits (1 hour each) per year
• Prescription management included without markup on any prescribed medication, including compounded medications. 
• Best for a patient who has stabilized enough to space out appointments but continues to require prescription management

• This is for a patient who has completed a foundational assessment , does not need intensive ongoing care, and requires a once or twice yearly appointment.
• The purpose of these annual or biannual visits is to:
  Review lab work, update supplement protocols, and determine
  individualized lifestyle interventions.
• This plan does not include portal messaging, concierge services, prescription management, or advanced lab testing.
