We will enroll you as a patient into our Electronic Medical Record manually within the next 24 hours.
Once you receive the welcome email, please complete the following steps:
- Log in to our Electronic Medical Record (Optimantra.com).
- Complete your intake forms and upload pertinent medical information.
- Your Provider will review your intake form responses and then order your lab work.
- Schedule your visit with your provider directly through the portal. Please allow at least 10 days for the labs to result.
